Required School Health Documents: Medical, Dental & Vision Forms

Newly admitted families must have their Permission to Release Confidential Records form submitted to the school within 7 days of their enrollment offer -and- any required updated Medical/Dental/Vision records in to the main office before the first day of school.
Returning families' updated forms are due to the school office before the first day of school. 

Immunization & Physical Form

In accordance with Illinois law, all children  beginning school or who are entering kindergarten and sixth grades  need to submit a current Immunization/Medical Examination  form.

Dental Form

In accordance with Illinois law, all children beginning school or who are entering into kindergarten, the second, and sixth grades need to submit a current Dental  Examination Form.

Vision Form

Illinois law requires that proof of an eye examination by an optometrist or physician (such as an ophthalmologist) who provides eye examinations be submitted to the school for students entering Kindergarten.

The AOA (American Optometry Association) recommends an eye exam every two years if no vision correction is required. Children who need eyeglasses or contact lenses should be examined every year.
